Jennifer Leis - Practice Manager 
Jennifer has been with Dayton Bariatric Center since 2003.  She was personally touched by the needs of bariatric patients, because she lost her own mother to obesity related problems when she was only 12 and feels that sensitivity towards patients is a must.   She has received a certification in  Medical Office Management and a certification in Medical Coding.   She has received ongoing  training in Advanced Coding, Leadership, Risk Management, HIPAA Security, Reimbursements for the Medical office, Compliance Officer, Collections and Patient Reimbursement, many ASMBS Advanced Practice courses, along with other areas required for compliancy as a Bariatric Center of Excellence and medical office.    She will work with you to familiarize you with the program and the necessity of your ongoing follow-up care for life-time.  She is available to discuss any questions or concerns you may have.

Tamika Foward - Nursing Assistant
Tamika has been with Dayton Bariatric Center since 2006.  She has previous experience with caring for the needs of patients in the home care setting.  She had bariatric surgery herself and knows first hand the concerns of our bariatric patients.   She cares for patients at their office visits and assists the doctors with patients ongoing care.  Her certification as a nursing assistant along with her experience assures patients of the importance of their healthcare to us.

Jackie Blank - Patient Support
Jackie has been with Dayton Bariatric Center since 2009.  She works with patients when they check in and preparing patients to meet with the doctor.  She is available to assist with questions you may have.
