Directions:
From I-75 or
I-675, exit to St. Rte. 725. Go East towards
Centerville.
Just past I-675,
turn left at the 2nd street on the left to Washington Village
Drive (Carvers Restaurant is on one corner and Chase Bank is on
the other)
Continue around
the bend and turn right into 7740 Washington Village Drive
at the "Cornerstone Medical Center".
Use the center
entrance and go to Suite 110.
